Team Leader
The Team Leader gives regular clinical and program supervision to 5 community support staff, leads team meetings and coordinates services within agency and with collateral supports. This position reports to the program director and is one of three Team Leaders. The agency serves 250+ men and women with serious mental illness each year and offers psychosocial rehabilitation, supported employment, and permanent supportive housing. We emphasize recovery and use evidence-based practices, including IMR, IPS, and CBT strategies. The Team Leader will serve a small number of individuals and may facilitate group services.
The Team Leader must be licensed (or very near licensure) to practice social work or counseling in Illinois and have a minimum of 3 years experience working with people with mental illness; must have a valid drivers license. Must know DHS Rule 132 services and documentation and have successful experience giving case management and other supports to men and women with mental illness.
Intake Coordinator
The Intake Coordinator determines applicants’ eligibility for services through clinical and psychosocial assessment and orients eligible clients and their families to the scope and nature of agency services. Assessments may occur at the agency or in the community. Efficiently and warmly responding to service inquiries is a significant portion of the work. This position composes a written profile and service needs summary with recommended initial services for use by the applicant and support team. The Intake Coordinator manages the state DMH registration process, data base and client case roster. We emphasize recovery and use evidence-based practices, including IMR, IPS, and CBT strategies. This position serves a small number of individuals and may facilitate a group service and reports to the program director.
